目的 当前手术中广泛使用髂前上棘(anterior superior iliac spine,ASIS)帮助定位股骨头中心(femoral head center,FHC)。本研究通过相关软件辅助测量CT后重建的骨盆和髋关节三维模型,了解ASIS、FHC与股骨大转子尖端(tip of greater trochanter,GT)在三维空间的位置关系。 方法 回顾性选取一定数量的成人骨盆和髋关节CT扫描图像,使用Mimics软件进行骨盆和髋关节三维重建,将生成的三维模型导入Geomagic Studio软件,标注ASIS、FHC和GT,在Pro Engineer软件中分别测量ASIS到FHC的距离(distance between ASIS and FHC,AF)、ASIS到GT的距离(distance between ASIS and GT,AG)、GT角(FHC与GT连线和AG之间的夹角)等数值,采用独立样本和配对t检验,研究各测量值差异。 结果 AF和AG的平均值分别为(79.34±7.12)mm和(88.65±9.14)mm,配对t检验结果显示,AF和AO[AO值等于AG×Sin(GT角)]值差异无统计学意义(P>0.05),FHC角(FHC与GT连线和AF之间的夹角)平均值为(91.04±11.68)°,且左右下肢差异无统计学意义(P>0.05)。 结论 虽然AF和AO值因个体不同而不同,但单个样本的AF和AO值接近相等,为骨科医生寻找一种较为有效、简便、准确的方法判断FHC提供了基础。
