摘要:
目的 应用无创压力-应变环(PSL)评估冠心病合并高尿酸血症患者的心肌做功情况。方法 回顾性分析2021年12月至2022年12月入住我院心内科行数字减影血管造影(DSA)确诊冠心病的119名患者,记录患者血清尿酸基线水平,根据血清尿酸水平分为高尿酸组(n=38)和正常尿酸组(n=81),两组均接受常规超声心动图检查并应用EchoPAC软件,输入血压值构建左心室PSL,获得心肌做功参数,包括整体做功指数(GWI)、整体有效做功(GCW)、整体无效做功(GWW)、整体做功效率(GWE),GWE=GCW/(GCW+GWW),比较各组上述参数的差异及与血清尿酸之间的相关性和回归性。结果 两组的GWI和GCW比较差异均有统计学意义(均P<0.05),高尿酸组的GWI和GCW均减小; Pearson相关分析表明,GWI、GCW与血尿酸值呈负相关(r=-0.255、-0.242,P<0.05)。线性回归分析表明,左室心肌做功值与冠心病患者的血清尿酸值存在线性相关。结论 无创压力-应变环可以量化高尿酸水平对冠心病患者的心肌损害情况,为临床提供指导价值。
Abstract:
Objective To evaluate myocardial work in patients with coronary heart disease(CHD)complicated with hyperuricemia using non-invasive pressure-strain loops(PSL). Methods A retrospective analysis was conducted on 119 patients with CHD who were admitted to the cardiology department of our hospital and diagnosed with digital subtraction angiography(DSA)from December 2021 to December 2022. The baseline serum uric acid levels of the patients were recorded, and they were divided into a high uric acid group(n=38)and a normal uric acid group(n=81)based on their serum uric acid levels. Both groups underwent routine echocardiography and utilized the EchoPAC software to construct left ventricle PSL by inputting blood pressure values. Myocardial work parameters were obtained, including global work index(GWI), global constructive work(GCW), global wasted work(GWW), and global work efficiency(GWE).GWE=GCW/(GCW+GWW). The differences in these parameters between the groups, as well as their correlation and regression with serum uric acid, were compared. Results There were statistically significant differences in GWI and GCW between the two groups(both P<0.05). Both GWI and GCW were reduced in the high uric acid group. Pearson correlation analysis showed that GWI and GCW were negatively correlated with serum uric acid values(r=-0.255,-0.242,P<0.05). Linear regression analysis showed a linear correlation between left ventricular myocardial work values and serum uric acid levels in patients with CHD. Conclusion Non-invasive pressure-strain loops can quantify the myocardial damage caused by high uric acid levels in patients with CHD, thus providing clinical guidance.
