摘要:
目的 研究乙酰肝素酶(heparanase,HPSE)及血管内皮生长因子(vascular endothelial growth factor,VEGF)在结直肠肿瘤患者组织中表达情况及临床意义。方法 选取经手术及病理证实为结直肠癌患者118列,采用免疫组化(Envison法)及逆转录聚合酶链反应(RT-PCR)半定量测定肿瘤组织、癌旁组织中HPSE及VEGF表达量,并分析其与一般情况、临床病理关系。结果 HPSE在结直肠肿瘤组织、癌旁组织中阳性表达分别为63.6%(75/118)和12.7%(15/118),VEGF在结直肠肿瘤组织、癌旁组织中阳性表达分别为71.2%(84/118)和17.8%(21/118),差异均有统计学意义(χ2=64.66,P<0.01; χ2=68.09,P<0.01)。结直肠肿瘤组织、癌旁组织中HPSE mRNA表达分别为0.223±0.062和0.093±0.026,VEGF mRNA表达分别为0.584±0.115和0.385±0.064,HPSE mRNA及VEGF mRNA在肿瘤组织表达水平均较癌旁组织明显升高(t=15.17,P<0.01; t=11.60,P<0.01)。HPSE、VEGF蛋白阳性表达和HPSE mRNA、VEGF mRNA表达量在患者年龄、性别、肿瘤位置、肿瘤大小方面比较,差异均无统计学意义(P>0.05); 但在肿瘤分化、肿瘤分期及淋巴转移方面比较,差异均有统计学意义(P<0.05)。结论 HPSE及VEGF在结直肠肿瘤患者的肿瘤组织中表达显著增高,在结直肠癌的发生、侵袭及转移中起着重要作用,可成为结直肠肿瘤患者预后判断的基础。
Abstract:
Objective To study the expression of heparanas(HPSE)and vascular endothelial growth factor(VEGF)in colorectal carcinoma and to investigate its clinical significance.Methods A total of 118 patients with colorectal carcinoma confirmed by surgery and postoperative pathology were selected,the expression of HPSE and VEGF in colorectal neoplasms tissues and in paracancerous tissues were detected by immunohistochemistry and real-time PCR.Their correlation with pathological characteristics and general information was compared.Results The positive expression of HPSE was 63.6%(75/118)in colorectal neoplasms tissues and 12.7%(15/118)in paracancerous tissues, the difference was statistically significant(χ2=64.66,P<0.01).The positive expression of VEGF was 71.2%(84/118)in colorectal neoplasms tissues and 17.8%(21/118)in paracancerous tissues, the difference was statistically significant(χ2=68.09,P<0.01).The expression of HPSE mRNA was 0.223±0.062 in colorectal neoplasms tissues and 0.093±0.026 in paracancerous tissues.The expression of VEGF mRNA was 0.584±0.115 in colorectal neoplasms tissues and 0.385±0.064 in paracancerous tissues.The expression levels of HPSE mRNA and VEGF mRNA in colorectal neoplasms tissues were significantly higher than those in paracancerous tissues(t=15.17,P<0.01; t=11.60,P<0.01).There were no significant differences in the expression of HPSE, VEGF protein and HPSE mRNA,VEGF mRNA in patients with age, gender, neoplasms location and neoplasms size(P>0.05), but the differences were statistically significant in neoplasms differentiation, neoplasms stage and lymph node metastasis(P<0.05).Conclusion The expression of HPSE and VEGF in neoplasms tissues of colorectal cancer patients is significantly increased,and plays an important role in the occurrence, invasion and metastasis of colorectal cancer.It might be used as the basis of screening,prognosis determination of colorectal carcinoma.
