摘要:
目的 探讨半乳糖凝集素3(Galectin-3)、细胞周期蛋白D1(Cyclin D1) 在甲状腺细胞学Bethesda-Ⅲ类结节中的表达及临床意义。 方法 对128例细胞学诊断为Bethesda-Ⅲ类结节的细针穿刺标本进行沉渣包埋,应用Envision法检测Galectin-3、Cyclin D1在标本中的表达情况。 结果 组织病理确诊为甲状腺乳头状癌(papillary thyroid carcinoma,PTC) 68例,良性病变60例,包括结节性甲状腺肿45例、滤泡性腺瘤11例、淋巴细胞甲状腺炎4例。在PTC中Galectin-3、Cyclin D1表达明显高于良性病变,阳性率分别为92.6%和89.7%,两者差异具有统计学意义(P<0.001)。两者联合检测可以提高诊断的敏感性、特异性、准确性,且在PTC与良性疾病中表达的差异具有统计学意义(P<0.001)。 结论 联合检测Galectin-3和Cyclin D1有助于甲状腺细胞学Ⅲ类结节的诊断与鉴别诊断,为临床治疗提供可靠的依据。
Abstract:
Objective To investigate the expression and clinical significance of Galectin-3 and Cyclin D1 in Bethesda-Ⅲ nodules of thyroid cytology. Methods A total of 128 specimens of fine needle aspiration with Bethesda-Ⅲ nodules of thyroid cytology were carried out of sediment embedding slice, and the expression of Galectin-3 and Cyclin D1 in the specimens was detected by Envision method.Results There were 68 cases of papillary thyroid carcinoma (PTC), and 60 cases of benign lesions, including 45 cases of nodular goiter, 11 cases of follicular adenoma, and 4 cases of lymphocytic thyroiditis. The expression of Galectin-3 and Cyclin D1 in PTC was significantly higher than that in benign lesion. The positive rates of Galectin-3 and Cyclin D1 were 92.6% and 89.7% respectively and the difference was statistically significant (P<0.001). The sensitivity, specificity and accuracy of the diagnosis were improved by the combination of the two tests, and the difference of combined expression between PTC and benign disease was statistically significant (P<0.001). Conclusion The combined detection of Galectin-3 and Cyclin D1 is helpful for the diagnosis and differential diagnosis of Bethesda-Ⅲ nodules of thyroid cytology, and provides reliable basis for clinical treatment.
