摘要:
目的 探讨脑侧支循环对急性脑梗死患者机械取栓术后转归的影响。方法 选取急性脑梗死机械取栓术患者106例,通过基于数字减影脑血管造影(DSA)的ASITN/SIR侧支循环评估系统评估患者基础侧支循环状态,并将患者分为两组,A组(48例)为侧支循环较差组,ASITN/SIR 0～1级; B组(58例)为侧支循环较好组,ASITN/SIR 2～4级。通过美国国立卫生研究院卒中量表(NIHSS)及改良 Rankin 评分量表(mRS)对患者术后神经功能进行评估,对比两组的神经功能转归,分析侧支循环对急性脑梗死机械取栓术后患者转归的影响。结果 侧支循环较好的急性脑梗死患者机械取栓术再通率高,患者术后出血转化率低,NIHSS评分低,mRS评分低,术后6个月内死亡率低,两组比较,差异有统计学意义(P<0.05)。结论 脑侧支循环较好的急性脑梗死患者机械取栓术后预后较好,脑侧支循环状态可作为急性脑梗死患者机械取栓术前进行预后评估的可靠的预测因素之一,良好的侧支循环可能成为血管内治疗时间窗延长的有利因素之一。
